The following Community Health Needs Assessment and Health Improvement Planning (CHNA & HIP) is a result of collaboration and partnership between Great River Health and public health departments to identify the community's greatest unmet healthcare needs.
The collaboration consists of an analysis of community health needs and assets and includes a set of priorities and objectives for taking action. Community participation for making decisions about improving the community’s health and mobilizing support and resources is required as part of this process.
We use these results to create a response to the identified needs, increase access to care, and improve the health of people living in the communities we serve.
