Home care is a comforting, cost-effective option to recovering in a long-term care center. Whether you are at home or in the hospital, our staff will work with you and your health care provider to decide if home care is an alternative. We help create a smooth transition from Southeast Iowa Regional Medical Center or another hospital or care center. Our treatment and your health care provider work together throughout your home care experience.

Services at this location

Our services are provided by a treatment team of registered nurses, home health aides, rehabilitation professionals and medical social workers who provide:

• Medication management
• Assisting with access of community and funding resources
• Disease management and education
• Evaluation visits
• Assessment of unstable and/or new medical conditions
• Maintenance care
• Post-operative care
• Blood draws
• IVs
• Ostomy care and teaching
• Physical, speech or occupational therapy
