Patient Portal

HCHC implements new patient portal October 1

On October 1, Henry County Health Center transitioned to Cerner, a new electronic medical record system. This is the same system used by Great River Health. This means that starting October 1, a new patient portal account needs to be set up with Cerner to continue to access all your patient portal benefits including health records, making an appointment, reviewing your prescriptions and communicating with your health care provider.

How to sign up

  • Starting October 1, provide your email address during patient registration.
    • Emails given before this date will not transfer over to the new portal so be sure to give your email address on October 1 or after that date.
    • After you give your email address, you will receive an email invitation to create your patient portal account. You can then sign up for the patient portal at www.greatriverhealth.org under the Patient Portal box.
  • You may also sign up for a patient portal in person at one of these locations:
    • Health Information Management
    • Patient Financial Services-Patient Billing
    • Patient Access
    • All Great River Health locations including Henry County Health Center, HCHC Rural Health Clinics, Southeast Iowa Regional Medical Center campuses.

To access your medical records prior to October 1, 2023, you can continue to access the MyCareCorner portal for one year. No new medical records after September 30, 2023, will populate in the old patient portal. All new medical records starting October 1 will go in the new patient portal.

If you currently have a patient portal account with Southeast Iowa Regional Medical Center, which is part of Great River Health, your HCHC information will now be available on that patient portal and you will not have to register or make any changes. The partnership between HCHC and Great River Health will enable you to have one patient portal for care received at all the clinics and hospital locations operated by Great River Health, including Mount Pleasant, Fort Madison and West Burlington.
